Saves a mishmash of local HIDE/SHOW defs along with various literals.
Also provide a function that computes the scale at which a given
IU size becomes (notionally) a certain size on screen. While this
is a simple division, it's a bit opaque in terms of meaning.
Also it means the divide by zero case can be more universally
defended-against, which has traditionally been a bug opportunity.
Recommendation is to avoid using the year nomenclature as this
information is already encoded in the git repo. Avoids needing to
repeatly update.
Also updates AUTHORS.txt from current repo with contributor names
This tidies up quite a performance hit on layer change
caused by repainting vias, pads and tracks for their
clearances.
Instead, on a layer change, just disable any old clearance
layer and enable the new one (if any).

This means that an early return or an exception between
a manual Push/Pop (or an omission of the Pop) cannot
corrupt the layer stack.
It also means the GAL doesn't have to maintain its own
stack (with the in-scope GAL_SCOPED_ATTRS taking that role).
Reomve the Push/PopDepth functions, as they're only ever
used in pairs, and doing it manually needs more care.

The hash table for integer hashes is extremely limited and places most
elements in the same buckets. This leads to a linear search time for
structures built on this.
This blocks hashes, directing the coder to utilize std::set or std::map
structures instead of hash tables for implementing integer-based
lookups.
New strategy isolates all draw/don't draw decisions to the ViewGetLOD
routines, and all dimmed/not dimmed to PCB_RENDER_SETTINGS::GetColor.
The actual drawing in PCB_PAINTER is more-or-less conditon free.
Also adds new layers for pad and via hole walls so that they can be
controlled for high-contrast mode.
Also changes the drawing paradigm so that the pads are drawn even when
not on the high contrast layer, just in low contrast. The hole wall
is drawn in high contrast. This actually makes things clearer to the
user (although to be honest was done to keep from having to re-render
pads when the high contrast layer changes since we have two separate
layers now that we can adjut colours on).

The problem with simple deleting VIEW_ITEM_DATA upon VIEW_ITEM removal was
caused by the default copy constructors that copied pointers.
Once a copy of an item was destroyed, the VIEW_ITEM_DATA has been
destroyed, effectively invalidating m_viewPrivData for the other item.
- Finalize transition to BOARD_COMMIT (removed all remaining uses of PICKED_ITEMS_LIST) and implicit view/ratsnest updates
- Simplified SELECTION class, it now can be directly added to a VIEW
- Removed unnecesary casts and templates
- Introduced C++11 features (range based for, lambdas) where they improve code readability
- Added non-undoable COMMITs, which can be used to propagate change notifications to interested listeners (e.g. ratsnest/view)
